Membership Requirements - Existing Meter and Service

A)  Application for Membership or Service
Complete an "Application for Membership or Service" form. If you download this file, the application must be printed on a legal sized (8.5" x 14") piece of paper and MUST be mailed in. We will not accept faxed copies of this document. All applications must be turned into one of our area offices and a photo ID is required before any service can be established. This application form requires one person's signature who is responsible for all bills and the correct mailing address. A wrong address can result in the power being disconnected and an additional service charge being assessed to have the account reconnected.
